If you do not know the percentage withheld from your pay check, divide the Arizona tax withheld by the Federal tax withheld.
The result is the Arizona Withholding Percentage.
(July 1 - Dec 31, 2010) Use 2nd table rates.
Check the box for Arizona and enter the appropriate percentage.
